1. Attend Pre-Membership Education Seminar. 
2. Fill out the Membership and Agreement Form. 
3. Attach 2 pcs. Of 2x2 Pictures. 
4. Attach Barangay Clearance and Photocopy of Birth Certificate (PSA or LCR Certified)
5. Attach a Photocopy of any Valid ID (Preferably, TIN) 
6. Pay Initial or Minimum Total Membership.
